If you reside in the United States and your Private Information was potentially impacted by the cyberattack on Communications Data Group, Inc.’s (“CDG”) network discovered in February 2025 (the “Data Incident”), including all those who were mailed a notification by or on behalf of the CDG, regarding the Data Incident (the “Settlement Class”), you may be a Settlement Class Member and eligible for Settlement Benefits. Excluded from the Settlement Class are: (a) all persons who are employees, directors, officers, and agents of Defendants, or their respective subsidiaries and affiliated companies; (b) governmental entities; and (c) the Judge(s) assigned to the Action and their immediate family, and Court staff.

If you are a Settlement Class Member and wish to receive Settlement Benefits then you must submit a claim form by November 10, 2026.

Class Members who submit a Claim are eligible to receive a Cash Payment for Documented Losses, an Alternative Cash Payment, or Credit Monitoring, all of which are described below:

Credit Monitoring. Class Members are eligible to enroll in three (3) years of Credit Monitoring offered by CyEx with one (1) major credit bureau (Experian, Equifax, or TransUnion), which will include at least identity theft insurance up to $1,000,000.00, without deductible.

Cash Payment for Documented Losses. Class Members are eligible to receive a Cash Payment for Documented Loss as reimbursement of up to $5,000.00 for money they lost or paid to protect themselves from the February 2025 Data Incident, such as money spent on a credit monitoring service, and as reimbursement for money they lost as a result of fraud or identity theft, if that money has not been reimbursed from another source. This includes without limitation and by way of example:

  • expenses associated with fraud or identity theft stemming from the Data Incident

  • professional fees including attorneys’ fees, accountants’ fees, and fees for credit repair services

  • costs associated with freezing or unfreezing credit with any credit reporting agency

  • credit monitoring costs that were incurred on or after mailing of the notice of data breach, through the date of claim submission

  • miscellaneous expenses such as notary, fax, postage, copying, mileage, and long-distance telephone charges

You must submit copies of documents supporting your claims, such as receipts or other documentation, and you must certify the information you submit on the claim form is accurate under penalty of perjury. “Self- prepared” documents, such as handwritten receipts, will not count as documentation, but you can submit them as clarification to other, official documents.

Alternative Cash Payment. In lieu of both a Cash Payment for Documented Loss and/or Credit Monitoring, Class Members may claim a $45.00 cash payment.

The deadline to submit a Claim Form online is November 10, 2026. If you are mailing your Claim Form, it must be mailed with a postmark date no later than November 10, 2026.
